Anesthesiologists

Responsible for all the monitoring and evaluation of the implementation of anesthesia, pre-anesthetic General itself and for monitoring postoperative, are an essential part in securing the safety of the procedure and in pain control, providing an immediate postoperative period quite comfortable.

Biopsies

The buccal cavity biopsies performed in the clinic may be total or partial character. They are performed in any kind of injury since characteristics that differ from normal patterns of the tissues of the oral cavity, be they hard tissue (bone) or soft (gums, mucous membranes, salivary glands, etc.

Bone Graft

For severely reabsorbed jaws and mandibles is indicated the bone graft of iliac crest in order to recover all the lost bone over the years with the absence of teeth.

Although some professionals have the opinion that this is a very invasive surgery for SEVERE defects of maxilla and mandible, bone rehabilitation is the most appropriate due to its cost-effectiveness. Fill the absence in bone resorption of this magnitude with biomaterials ends up being a very costly alternative because of the cost of this type of such bone substitute in Brazil. The biomaterials are ideal for bone reconstructions of light and moderate resorption.

With proper planning and the advice of our multidisciplinary team the patient performs this surgery in a very safe and predictable way. Besides the advantage of being able to provide as material donor your own bone, which is rich in osteoconductive and osteoinductive cells from your own organism (autogenous bone).

Facial Trauma

Face trauma can be caused by several reasons, including these: traffic accidents, physical assaults, falls or even during the fun (football match, participation in endurance events, racing, etc.) At this point, some or several bones of the face can be fractured, causing signs and symptoms that are not always perceived by the patient or by other professionals other than an oral and maxillofacial surgeon.

High-tech Imaging tests available in the region in help a correct diagnosis and a pre-surgery planning that leads up to the best forms of treatment, which should be conducted under general anesthesia so that the fractures can be repositioned to its original position and fixed with a clinical and titanium mini.

For these cases we also offer multidisciplinary team of our clinic, because when it comes to the complexity of anatomical structures that involve the bones of the face, nutritional and psychological follow-up is necessary most of the time.

General Anesthesia

The procedures requiring general anesthesia are performed at the Hospital São João Batista (www.hsjb.com.br), in the city of Criciúma-Santa Catarina.

We have a team of highly trained Anesthesiologists, responsible for providing safe procedures and a personalized postoperative patient care, fully integrated to our multidisciplinary team.

Implants

The implants were developed to be the replacements of the roots of the teeth for some reason they must be condemned. They replace in a natural way the missing tooth. Because they are made of pure titanium do not cause rejection, because this metal is 100% biocompatible to human body.

Keeping the philosophy of  exceed by using the most modern and effective in dentistry, the clinic works only with the German brand Ankylos implant. Since the Foundation of the implant clinic elected to knitting of clinical cases is the Ankylos ™ because stability and beauty that the quality and the design that this implant offer.

Many cases of rehabilitation of dental implant absences, should have the bone and gingival portion retrieved previously or concurrently to the implant. When not diagnosed these absences, the results can be catastrophic.

Emphasizes, so that is always very important a correct diagnosis and planning of clinical cases by the professional, who will lead you through an accurate clinical examination and Imaging tests with advanced technology..

Jugal lipectomy (removing the Ball of Bichat)

The ball of Bichat or bag of Bichat is a fatty deposit (FAT) that all human beings have the face, in the region of the cheeks, located between the muscles of mastication masseter and Buccinator. It serves as a cushion, to lubricate the interposition of these two muscles and fill the space of the cheeks of the face.

However, for patients who have a very round face, fat, wide or with the outline of the face bit set, the withdrawal of this fat is indicated. Even thin patients can look like round face, because the ball of Bichat is not linked to obesity, she is an anatomical organ, therefore, does not disappear when we lose weight.

Its withdrawal (Bichatectomia or Lipectomy Jugal) is a relatively simple surgical procedure, without great complexity, which gives patients a face with the malar (cheekbone) and most protruding cheeks look thinner and inside, since fat is excised (removed) completely through an intra oral access, that is, there are no scars on the face because the procedure is performed inside the mouth.

As only the fat is removed, having no concomitant muscle level intervention, the aesthetic effect is very nice and discreet, because sets and thins the face of the patient without any scars. Is a cosmetic procedure routine, safe, which can be performed under local anesthesia (at the clinic) and general anesthesia when other interventions are associated with such as:

1) Mentoplasty (Chin plastic);
2) surgery to emphasise the mandibular angle through the installation of Porex Mandibular prosthesis;
3) orthognathic surgery;
4) surgery to disguise the Naso-Labial Groove deep through the installation of Porex prosthesis for Paranasal Region;
5) Other cosmetic surgeries in the Face (one can associate the presence of two surgeons in the procedure – oral and maxillofacial Surgeon + plastic surgeon).

Mentoplasty (Chin plastic)

For patients who do not like their profile and do not have indication for orthognathic surgery, mentoplasty or plastic chin surgery is a safe and effective procedure for the correction of the profile. It is indicated:

** When the patient presents double chin, i.e. the lack of jaw causes the submental fat startling giving the appearance of a double chin.

** When the patient has very long Chin, in this case a reduction in height is the most appropriate.

** When the patient presents Chin too short, causing a Groove ment-labial too deep, in this case an increase in the height of the Chin indicated.

** When the patient presents Chin too far forward, in this case an indentation of the Chin is the most appropriate.

Important to note, that any type of prosthesis is used in this type of plastic surgery. Ment of the patient himself is cut and fixed in the desired position with the help of-plates and titanium mini (see photo).

The patient should always check the predictive path that the surgeon will make your face, in virtue of which the Chin plastic, when scarcely indicated, can have catastrophic consequences on the harmony of a face.

Nutritionists

Responsible to plan and indicate a specific diet for the post, but in a personalized way, according to the characteristics and the tastes of each patient.

Oral and maxillofacial Surgeon

Responsible for DIAGNOSIS, INDICATION, PLANNING, PROPER EXECUTION AND MONITORING POSTOPERATIVE IN THE SHORT, MEDIUM AND LONG TERM patients who will undergo interventions in maxillofacial region.

Orthognathic surgery

Orthognathic surgery, as its name suggests (orthos = correct/gnathus = jaw) is a surgery performed on the middle and lower thirds of the face to fix the defective placement of the bones of this region. Despite being a technically complex surgery, it can be performed with a lot of predictability and security.

A multidisciplinary team composed of anesthesiologists, oral and maxillofacial surgeon, nurse, nutritionist, orthodontist and psychologist gives patients a pre, trans and post-operative, quiet and comfortable.

This type of surgery is indicated for patients who have any type of discrepancy between the maxilla and mandible, either in a mild, moderate or severe form, causing unclamping, i.e. the bite does not fit properly. The patient shows a change in the whole physiology of the stomatognathic system: speech, swallowing, breathing, and chewing. Only the surgery restores normality to these functions.

Another indication is given to patients who have the obstructive sleep apnea syndrome. In these cases, the advance of the maxilla and mandible that provides the orthognathic surgery, has the ability to eliminate the obstruction in the airway later, by the fact that all the muscles of this region moves forward at the same time to the advancement of the bones.

The clinical treatment of obstructive sleep apnea with mandibular advancement plates depends on the use of it. At the moment the patient suspends the use of the plates occurs the return of the episodes of apnea. Therefore, the orthognathic surgery in these cases is the definitive treatment.

Orthopedic Surgeon

Responsible for the patient's preoperative assessment with focus on donor region of bone from the iliac crest. Use this bone in the region of the hips as donor area when the reconstruction of severe resorption of maxilla and mandible severely atrophied.

Orthopedic Surgeon

Responsible for the patient's preoperative assessment with focus on donor region of bone from the iliac crest. Use this bone in the region of the hips as donor area when the reconstruction of severe resorption of maxilla and mandible severely atrophied.

Periodontal Plastic Surgery

Periodontal plastic surgery is indicated for patients who have a smile with a lot of exposure of gum or have a gap between the gingival margin of the teeth.

This technique has been used a lot lately in virtue of which provides an increase in dental size up, leaving the more harmonious and natural smile. This is a surgery under local anesthesia, held his own clinic. Always occurs a virtual planning for the patient to view the result of your smile through a simulation of the same.

In some cases, there is a tooth under the gum and plastic only is indicated. When the tooth root is exposed after plastic surgery, indicated the achievement of porcelain veneers to the cover-up of the teeth.

However, severe gingival smile cases can be dealt with only through orthognathic surgery.

Phonoaudiologist

Responsible to approach the whole issue of oral motor function in the pre and in the postoperative in surgery, as orthognathic, which provides a considerable facial change, the phonoaudiologist is essential in monitoring before and after the operation, because this procedure repositions the entire musculature of the the face and the oral cavity from the bone correction provided by surgery.

The exercises are passed on an individual way for each patient, anticipating the recovery of it. A lymphatic drainage of soft tissues is made by minimizing the post-operative swelling, restoring more quickly and effectively the muscles of the operated site, causing the region regain its normal function and aesthetics.

Preparatory procedures for surgery

For the convenience and comfort of the patients we provide a "pre-operative Afternoon", an afternoon period that the patient spends in our clinic and makes all the routine that a preoperative requires, in just one time and just one place.

1) pre-anesthetic evaluation with the team of Anesthesiologists
2) nutritional evaluation with the Nutrition team
3) psychological evaluation with psychologists team
4) Orthopedic evaluation with Orthopedic team (when necessary)

Rehabilitation prostheses for large extensions with absence of tissues in the maxillofacial region

Patients who have suffered major traumatic injuries, have undergone resection of large or medium-sized tumors with different degrees of malignancy, or that present congenital malformation in the facial maxillary region, find in the clinic a service for the rehabilitation of the missing portions of tissues that are usually present after the occurrence of one of the factors listed above, through the manufacture of large extensions prostheses, planned and custom-made for each client.

The clinical case will be analyzed in detail. Complementary examinations, such as tomography and 3D (three-dimensional) virtual analysis, will be requested for the ideal planning and prosthetic rehabilitation of the clinical case in question.

Skeletal anchorage (bone supports) through several available models, can be used to support the prosthesis, that will rehabilitate all tissues absent extension (bone, muscle, dental, gingival tissue, etc.).

In this way, the patient can be reintegrated into social life with discreet, customized and beautiful prostheses, and most importantly, extremely comfortable for use in primordial daily activities such as chewing, swallowing, breathing and phonation (speech).

Surgery for obstructive sleep apnea syndrome

The obstructive sleep apnea syndrome is a syndrome characterized by snoring, heavy breathing and pauses in breathing during the night due to a subsequent upper airway obstruction. The cause is a laxity in this region, causing the collapse of same and preventing the passage of air to the lungs. It is characterized by several signs and symptoms:

+ Attention deficit
+ Headaches (especially upon waking up)
+ Daytime sleepiness (regardless of the number of hours sleep)
+ Snoring and labored breathing
+ Increased blood pressure
+ Lethargy, illness and fatigue
+ Dry throat when I wake up
+ Sore throat
+ Light sleeper
+ Depression

Treatment includes weight loss, use of devices to make the positive air pressure (CPAP) overnight and which provide a mandibular advancement devices. However, some patients, especially those with severe apnea, can have their case resolved only through advance surgery of cheek bones (orthognathic surgery).

Despite the breakthrough surgery of cheek bones be technically similar to orthognathic surgery, you can ride with no aesthetic to the face of the patient, i.e. There is no significant changes in the appearance of the face.

It is a safe procedure, performed at hospital level, but with the involvement of all our multidisciplinary team: anesthesiologist, oral and maxillofacial surgeon, psychologists, nutritionists and nursing staff.

The clinical results are rewarding and provide well-being and quality of life for patients.

Surgery of oral pathology

Is understood by surgery of the oral pathologies any surgery conducted in the oral cavity for the removal of structures that interfere in patient´s well-being and in the conduct of treatments conducted by other professionals working in this region: Orthodontists, head and neck surgeons, Periodontists, etc.

Among these procedures include removal of lingual and labial brakes, removed or teeth and impacted include tractor, tooth extractions, drainage of abscesses, etc. This subdivision includes the definitive treatment of the lesions were biopsied from a partial form: cysts and tumors (benign or malignant) odontogenic source or not, Dysplasia of the oral cavity, etc.

All pathologies of hard and soft tissues of the oral and maxillofacial region, including impacted teeth.

Surgery to emphasize the contour Mandibular

This surgery is performed under the aesthetic advantages it provides in the face of the patient of a discreetly, anatomical and natural.

It is a German pre-formed polypropylene prosthesis in several sizes to be embedded in the bone of choice, providing an outline of a beautiful and elegant way. These prostheses fill the spaces causing a stretching of the skin preventing plastic surgery for facial lifting.

Without any scars extra-oral (surgery is all held inside of the mouth), the prostheses are fitted in place of choice and by having micro-porous parts are absolutely natural in relation to underlying tissues, due to the incorporation of the prosthesis.

You can leave the outline of the mandibular angle more prominent, increase the zygomatic region (cheekbone), reduce grooves ment-labial and naso-labial very deep, etc.

Unlike the fills, the prostheses provide a definite result, not requiring maintenance (However, is subject to removal if the patient does not feel satisfied with the result).

Surgery to Increase the Volume of the Paranasal Region

For patients that have the paranasal nasolabial Groove region deep, causing a premature aging of the face by the presence of nasolabial Groove very marked, surgery is indicated for installation of Porex prosthesis to increase the Volume of the Paranasal Region.

The installation of the graft (which has various sizes) causes an increase in volume at the site, stretching and projecting the soft tissues.

The patient rejuvenates, improving the appearance of the final form, through a surgical procedure one time, without having to undergo constantly fillers in the region of nasolabial Groove, which must be performed each time the tissue volume disappears with absorption of filling by the body.

One of the advantages of this prosthesis of Porex paranasal region is that it increases the volume of the region, causing positive changes paranasal but discreet on the nose, unlike jaw advancement through orthognathic surgery, which causes positive changes, but significant in the nose.
So is the perfect option for patients who have no indication of orthognathic surgery or have a mild or moderate subsidence of paranasal region who would like to have a more natural-looking, beautiful, young and harmonious.

Patients who have cleft lip (cleft) and do not need to undergo the orthognathic surgery or who would like to have the aspect of his face improved while awaiting the correct age to perform the surgery, can have this procedure temporarily (stay with the prosthesis until the moment of orthognathic surgery) or permanent (do not remove the prosthesis, because it does not have the need or other surgical procedures on the face).

Temporomandibular joint (TMJ) Disorders

The surgery of the temporomandibular joint (TMJ) is indicated when there are problems in one or more components of these joints, proven through diagnosis with magnetic resonance imaging (MRI). Similar to other joints of the human body, such as knee, shoulder, the components that make up the temporomandibular joint can be injured and only be liable for a recovery through the surgical treatment.

Many professionals treat disorders of TMJ clinical form only via muscle relaxants cards to be used by the patient during a good part of the day, this treatment is correct if the diagnosis of the condition that involves the articulation is also correct. In some cases, the structures of these joints are so injured and displaced the treatment for lead to pathology installed on ATM is only and exclusively surgical.
